Changes to middle school athletic participation form requirements
An updated Athletic registration form and a new Concussion Awareness form is required for middle school athletic participation beginning with the 2009-2010 school year and Inherent Risk forms are also required for each sport in which a student athlete participates.
If you downloaded a middle school sports registration form before July 15, be sure to download a new one as we've updated Section III: Injury Risk/Parent Permission. You will also need to download a Concussion Awareness form and an Inherent Risk form for each sport you choose. These new forms must accompany the middle school sports registration form.
Please turn in completed athletic forms to your school office.
Middle school sports registration form
To complete the middle school sports registration form:
- Parents complete Sections I, II, and III.
- Students and parents complete Section IV.
- Physician completes Section V. Signed originals are required. We cannot accept faxes or photocopies of physical forms.
- Print, read and sign a Concussion Awareness form (below).
- Print, read and sign an Inherent Risk form (below) for each sport in which the student athlete participates.
