  • Access to Athletic Forms
    For any student intending to participate in middle school athletics, there are a number of required forms that need to be on file in the office prior to participation.
  • Administration of oral medication at school
    Washington State law and Issaquah School District policy require that schools have written authorization from a licensed healthcare provider and parent on file yearly when a child needs to take prescription or over-the-counter medication during the school day. Please fill out this form if it pertains to your child.
  • Administration of medication for 72 hours at school (for disaster planning)
    In the event of a major disaster, schools may be responsible for the care of students for as long as 72 hours. If your student is taking medications for diabetes, seizures, heart or serious behavior problems on a regular basis, please fill out a 72 hour medication form which includes parent and licensed health care provider information, and bring the medication to school in a labeled prescription bottle.
  • Free and Reduced Lunch application
    Complete this application if you would like to apply for free or reduced price lunches at school. Only one application per family is required. If approved during the last school year, your student's status briefly carries over into the new school year; however you must submit a new application annually to continue receiving benefits.
  • Residence Verification form
    Please use this form if you have relocated. This completed form along with proof of residence is required.

FERPA / Family Privacy Options

The Issaquah School District needs to honor families' privacy while meeting its obligation to share education-related information with the community. The annual Family Education Rights and Privacy Act (FERPA) notification and opt-out process helps schools balance these interests. Please read the following FERPA information sheet carefully. No response is required unless you want to opt your student out.
