• Confidential Emergency Health Information Form
  All students are required to complete this two-page form every school year. Complete as much information as possible, as these forms are utilized when students experience a medical emergency. A parent/guardian signature is required.
• Electronic Information System Individual User Access Informed Consent and Release Form
  Required to use computers and access the Internet at Maywood Middle School. Parent/guardian and student signatures are required on this form.

FERPA / Family Privacy Options

The Issaquah School District needs to honor families' privacy while meeting its obligation to share education-related information with the community. The annual Family Education Rights and Privacy Act (FERPA) notification and opt-out process helps schools balance these interests. Please read the following FERPA information sheet carefully. No response is required unless you want to opt your student out.
