Do You Have a Student Entering 7th Grade in the Fall?
Tdap is a Washington State immunization requirement for students to enter 7th grade
School must have a medically verified date of Tdap vaccine on file in order for your student to begin school.
To update your child’s Tdap immunization record at school, you will need to turn in a medically verified immunization record. Your child’s health care provider can print this for you, or you can print it from MyIR,
which is a free Department of Health online tool that allows families to view and print their official immunization records themselves. Go to https://wa.myir.net/register to begin the sign-up process.
Please send the medically verified date of Tdap for your student to the nurse Nina O'Brien at Maywood, firstname.lastname@example.org, fax to (425) 837-6910 or call (425) 837-6905 if you have questions.
If your child has a valid exemption on file at school for Tdap, you do not need to do anything.
