Do You Have a Student Entering 7th Grade in the Fall?
Tdap is a Washington State immunization requirement for students entering 7th grade. The school must have a medically verified date of Tdap vaccination on file in order for your student to begin school.
To update your child's Tdap immunization record at school, you will need to turn in a medically verified immunization record. Your child's health care prover can print this for you, or you can print it from MyIR, which is a free Department of Health online tool that allows families to view and print their official immunization records themselves. Go to https://wa.myir.net/register to begin the sign-up process.
Please send the medically verified dat of Tdap vaccination for your student to the nurse at firstname.lastname@example.org.
If your child has a valid exemption on file at school for Tdap, you do not need to do anything.
