Student Ages 5-11 COVID Vaccine Clinic

Nov. 5 update: On Nov. 2, we sent a message inviting families to a free, optional COVID-19 vaccine clinic for students ages 5 to 11. Interest in this clinic was extremely high, and all appointments were booked within 25 minutes. On Nov. 4, Costco was able to add another 700 appointments to the clinic, and we emailed families to let them know about the opportunity. Those new appointments were gone within 18 minutes.
We are grateful for the opportunity to partner with Costco to provide this service to those of you who have already booked appointments. For families who were unable to secure an appointment before they filled, please know that while we cannot make any promises, we are investigating the possibility of offering additional dates. If Costco and the District are able to offer additional vaccination opportunities, we will send an update to families and staff.

We will host a free, optional COVID -19 Pfizer vaccine clinic for our students, in partnership with Costco. Students must be between the ages of 5 and 11 by Nov. 10, 2021, in order to be eligible for this vaccine clinic.


First dose:

  • Wednesday, Nov. 10: 2:30-7:30 p.m.
  • Thursday, Nov. 11: 3:30-8:30 p.m. (Nov. 11 is a non-student day in observance of Veterans Day).
  • Friday, Nov. 12: 3:30-8:30 p.m.
  • Saturday, Nov. 20: 8 a.m.-1 p.m.

Second dose:

  • Wednesday, Dec. 1: 2:30-7:30 p.m. (for those who received dose 1 on Nov. 10)
  • Thursday, Dec. 2: 3:30-8:30 p.m. (for those who received dose 1 on Nov. 11)
  • Friday, Dec. 3: 3:30-8:30 p.m. (for those who received dose 1 on Nov. 12)
  • Saturday, Dec. 11: 8 a.m.-1 p.m. (for those who received dose 1 on Nov. 20)

Please note: The second dose appointment will be automatically scheduled for the same time of day, three weeks from the date and time of the student's first dose. A scheduling notification may not be sent for the second appointment.

Families will receive a confirmation email with additional check-in information, including a link to a consent form. Please print, sign, and bring a physical copy of this form to the vaccine clinic for each child receiving a vaccination. Note: some of the information on the printed form will be the same as what you filled out online.

If you need to cancel or reschedule an appointment, please click the active link at the bottom of your original confirmation email. If you need further assistance, please email and a Costco staff member will assist you.

Important things to know the day of:

  • The vaccine is free. While insurance is not required, if you do have insurance, please bring your card to your appointment and provide your information on the signed consent form.
  • One parent or guardian must be present at the clinic with their student(s). Only one parent/guardian will be allowed to accompany each child inside the clinic.
  • To lower the risk of the spread of COVID-19, upon arrival, we will take your temperature and ask you questions about COVID-19 symptoms, exposures, and travel.
  • Masks are required.
