Sports Info for the 2021-22 School Year


Will your student be participating in the ISD sports programs offered at middle school?  If so, mark your calendar and schedule their sports physical during the summer.   Students entering 6th grade that plan to play sports are required to have a physical date after June 1.   A physical date in June, July or August is valid for the entire school year.  <Click here> for the physical form for the doctor's office to complete.


It is currently our plan to offer our normal middle school sports programming when we return for the 2021-22 school year.  June, July, and August are the perfect time to schedule your student’s annual physical.  Summertime physicals are valid for the entire school year.  All incoming sixth graders must have a physical date after June 1st.  All 8th graders transitioning to high school will need a physical date after June 1st to participate in high school sports programs.  Contact Casey Stookey at stookeyc@issaquah.wednet.edu if you have any questions about sports or physicals. 


In August, all incoming 6th grade families will receive an automated email from FinalForms.  (FinalForms is our online sports registration provider.)  The email will ask parents to confirm their account with FinalForms.  If your student plans to participate in ISD Middle School sports you will want to complete the account confirmation process and save your user ID/Password for future use.   If you are unsure, at least save the email.  If your current 5th grade student has older siblings in the district you have already received this account confirmation email from FinalForms.
