Please print, sign and return forms on the first day of school September 6, 2017.  These will be collected during the picture taking process.  To obtain your ID/ ASB card all 3 required forms must be turned in. Juniors and Seniors may also turn in their lunch release form at this time. Forms WILL NOT be collected in August!

    • Parent Signature for Student Handbook
      Our goal is to provide a safe and caring environment, where all students experience a sense of caring and feel connected to their school. The Student Handbook, including our policies, procedures and important information, helps to ensure our students have an optimal high school experience.
    • Confidential Emergency Health Information Form
      All students are required to complete this two-page form every school year. Complete as much information as possible, as these forms are utilized when students experience a medical emergency. A parent/guardian signature is required.
    • 6-12 Students Responsible Use Agreement (Internet Acceptable Use Policy)
      Required to use computers and access the Internet at Liberty High School. Parent/guardian and student signatures are required on this form.
    • Issaquah School District Parent Attendance Letter
      A parent/guardian signature is required on this form.

Optional school forms

  • Athletic Registration Process
    Students intending to participate in Cheer, Drill, or a high school sport must complete an athletic eligibility process in order to receive clearance.   • Authorization for Administration of Medication at High School
    Required for any students that need medications during school hours.
  • Administration of medication for 72 hours at school (for disaster planning)
    In the event of a major disaster, schools may be responsible for the care of students for as long as 72 hours. If your student is taking medications for diabetes, seizures, heart or serious behavior problems on a regular basis, please fill out a 72 hour medication form which includes parent and licensed health care provider information, and bring the medication to school in a labeled prescription bottle.
  • Free and Reduced Lunch Application
    Click the link above if you would like to apply for free or reduced price lunches at school. Only one application per family is required. If approved during the last school year, your student's status briefly carries over into the new school year; however you must submit a new application annually to continue receiving benefits. If you would prefer a paper copy of the application, they are available here.
  • Lunch Release Form
    This form is required for Juniors or Seniors only who would like to leave campus during their assigned lunch period. A parent/guardian signature is required. This form may be turned in on the first day of school with the required forms packet.
  • Parking Lot Rules and Vehicle Registration Form
    If you are planning to drive to Liberty High school this year, please read this form with your parents and sign in the appropriate area. This completed form must be presented to receive a parking permit.
  • Residence Verification form
    Please use this form if you have relocated. This completed form along with proof of residence is required.
  • McKinney Vento Student Residency Questionnaire
    Complete this form if you are enrolling a student that is homeless under the definition of McKinney-Vento (without "fixed, regular, and adequate nighttime residence".) This generally means that a child or youth does not have a stable place to sleep at night. Completion of this form will assist the school in identifying students that may need additional services and resources.

FERPA / Family Privacy Options

The Issaquah School District needs to honor families' privacy while meeting its obligation to share education-related information with the community. The annual Family Education Rights and Privacy Act (FERPA) notification and opt-out process helps schools balance these interests. Please read the following FERPA information sheet carefully. No response is required unless you want to opt your student out.
